Application Information

The Louisiana School Application Forms may be obtained from a variety of sources including your school counselor, the LSMSA Admissions office, and this LSMSA web page. You may obtain a printed copy of the LSMSA Admission Forms from LSMSA by calling 1-800 259-3173. You may download the complete application in PDF format by selecting the appropriate link on this page. The application forms in MS Word format may be opened and saved on your computer by following the appropriate links of this page. The MS Word document enables you to complete the form on the computer before printing. Each part of the application may be submitted at any time. The application deadline is February 1st. The LSMSA Application requires either an ACT or SAT test score before. Register for testing through your school counseling office or through the ACT or SAT website. LSMSA will accept results of the February 9 ACT testing and the January 26 SAT testing.
